Contracting
By a written contract, each dentist agrees to provide quality care to Delta Dental of Alaska members. The dentist also agrees to prefile fees and accept direct reimbursement from Delta Dental of Alaska.
Rules for Participating Dentists
Participating dentists agree to abide by the following rules of Delta Dental of Alaska, in addition to other rules established and set forth by the Delta Dental of Alaska Board of Directors:
- To submit a completed ADA standard treatment form without charge to the patient.
- To accept Delta Dental of Alaska benefit payments for services provided.
- To submit a listing of usual fees to be filed with Delta Dental of Alaska for payment of dental services provided to Delta Dental of Alaska covered patients. Any change in fee schedules is limited to once every six months.
- To keep accurate and complete financial and patient records in a manner that meets generally accepted practices.
- To allow Delta Dental of Alaska access at reasonable times upon request to inspect and make copies of the books, records, and papers of a Participating Dentist relating to the dentist's fees charged to all his or her patients, to the services provided to patients, and to payments received by the dentist from such patients.
- To not charge a Delta Dental of Alaska patient an amount over the copay, deductible, or the dentist's accepted fee.
- To not submit charges to Delta Dental of Alaska for payment for treatment which is not completed.
- To not submit charges to Delta Dental of Alaska for services provided to the dentist's family members or employees.
- To provide accurate and complete information to Delta Dental of Alaska.
Credentialing Requirements
Delta Dental of Alaska maintains a file of your credentialing information. The Credentialing Form needs to be completed and returned to Delta Dental of Alaska as a part of your participating agreement. Delta Dental of Alaska will verify your credentials every three years. Recredentialing Form
