Getting Started with Employer Online Services
To set up access, please follow these steps:
- Medical groups must complete and sign the Employer Online
Service Agreement. This is not necessary for Dental groups as it is included in your contract.
- Fax the agreement to the EOS Web Administrator at 503-243-4498.
- The Web Administrator will call you with your login
and password.
